PUBLIC NOTICE: HIPAA Breach Notification at Valley View Health Center's Raymond Clinic
(July 11, 2014) Valley View Health Center takes the privacy of our patients very seriously. We recently discovered that certain personal information we maintain as a part of private health records was accessed inappropriately by one of our employees without our knowledge or authorization. This employee has been dismissed. Under the rules established by the Health Insurance Portability and Accountability Act (HIPAA) it has been determined that a breach of some Protected Health Information has occurred.
We have reported this incident to the Department of Health & Human Services’ Office of Civil Rights. We are reviewing our systems, procedures, and protocols and will implement any changes necessary to ensure that further breaches are avoided. We apologize for any inconvenience or concern this incident might have caused you.
